Job Description

Responsibilities:
Provides comprehensive assessment, recreation therapy and socialization and coordination of 24 hour care for PACE participants as part of the PACE interdisciplinary team. Plans, organizes and conducts a comprehensive program of individual and group recreation to enable participants to achieve optimum levels of independent functioning and socialization.
Qualifications:
Two years of experience in a social or recreational setting providing and coordinating service for elderly populations within the last five years, one of which was full time in a patient activities program in a health care setting. Bilingual candidate is highly desired
Education:
Bachelor's Degree in recreational or activities therapy, music therapy or other therapeutic activities area required.
