Housing Assistance Supervisor Description:

The SOAR (SSI/SSDI Outreach, Access, and Recovery) Specialist will assess referred participants from HHSI's Housing Programs to determine their eligibility for Social Security disability benefits. For those who qualify, the specialist will provide assistance throughout the application process. This role will manage a full caseload of individuals at various stages of benefit application.

Qualifications:

Education:

Must have at least a high school diploma and/or GED with 2 years of social services experience preferred.

Experience:

Strong writing background demonstrated through 1-3 years of professional experience in technical drafting, case management documentation, or legal correspondence. Two years of experience in social services is preferred. Excellent written and verbal communication skills, with the ability to draft clear client correspondence, appeals, and application for Social Security benefit determinations. The ideal candidate will have strong, persuasive writing skills with an emphasis on clarity and accuracy; proofreading skills and creativity are highly valued. Applicants must demonstrate experience in crisis intervention, Trauma-Informed Care, and social work practices specifically with individuals experiencing homelessness and/or those living with disabilities, including mental illness, substance use disorders, physical or developmental disabilities, and chronic health conditions. A commitment to maintaining the confidentiality of employee and participant records is essential.

Essential Duties:

Work with referrals from HHSI interagency program. Complete interviews to gather information to complete

SSI/SSDI

applications. Gather medical records and other information to complete

SSI/SSDI

applications. Write SOAR Medical Summary Reports for individual applications. Accompany individuals to appointments at the Social Security Administration (SSA). Coordinate visits to medical doctors, psychiatrists, and other specialists to obtain evidence for the application. Coordinate additional services with partners and help with providing care coordination services to individuals when needed. Communicate with key community partners (e.g. SSA, Disability Determination Services, and medical providers). Assist the team with administrative tasks as needed. Report outcomes in the SOAR Online Application Tracking (OAT) system. May serve as a payee on behalf of the agency for individuals enrolled in the program 3.

Other Duties:

Collaborates on case plans for high need participants with the Supervisor Maintains knowledge of Evidenced Based Practices for specific program Maintains strong knowledge of community resources, especially as it relates to housing Regularly promotes the agencies' mission and advocates for social justice Performs other duties, as needed

Schedule:

Full-Time. Must be able to work 36 hours per week. Position is an in-office position working in person with clients and coming to the office. Working hours are first shift between 7am and 4pm, but flexible in knowing that some nighttime hours may be required to connect with participants.
