Up to $100,000+ annually (Base salary + Performance Incentives) Are you a relationship-driven healthcare professional with a passion for connecting patients to quality post-acute care? Oakmont Guest Care Center is seeking an experienced Clinical Liaison to develop referral relationships and help patients transition successfully from the hospital to skilled nursing. What You'll Do Build and maintain strong relationships with hospitals, physicians, case managers, and discharge planners. Conduct on-site hospital assessments and coordinate admissions. Educate referral sources on Oakmont's clinical programs and services. Collaborate with the admissions and interdisciplinary teams to ensure smooth transitions. Meet and exceed census growth goals while maintaining exceptional customer service. Represent Oakmont professionally throughout the DFW healthcare community. Qualifications Clinical background preferred (RN, LVN, PTA, OTA, RT, Social Work, or similar healthcare experience). Skilled nursing, rehabilitation, hospital, LTAC, or home health marketing experience preferred. Established referral relationships within the DFW market are a plus. Excellent communication, organization, and relationship-building skills. Valid driver's license and reliable transportation.
