This range represents the organization's good faith estimate of the possible compensation at the time of posting.POSITION SUMMARY
: Infectious Disease Program Manager is responsible for working directly with the Infectious Disease team. This Manager serves to deliver comprehensive care to patients living with HIV, Hep C, TB, and other infectious diseases. The Infectious Disease Program Manger along with Infectious Disease provider, lead the clinical team in the standard of HIV care. The Manager is responsible for personnel management including hiring, supervision, evaluations, and terminating. Provides Supervision of STI Clinic Staff and HIV Medical Case Managers and day-to-day operations. Monitor caseloads & charts; rates of completion of Individual Service Plans, and all other aspects of Medical Care Coordination activities conducted by the team. The Manager also completes DPH reports monthly and annually and maintains the State and Federal CAREWare database. Reports on grants, organizing team and patient meetings, handles coordination of procedures, delivery of specimens, assists the providers, works with the ID nurse collaborating care for our high-risk patients; handles triages; phone calls; orders medication; gives direction to the MAs, and Medical Case Managers. This individual oversees the STI walk in clinic and HIV team workflows and collaborates and assists with the Infectious disease cases in the Pediatric, OB/GYN and HRC departments. This individual is also responsible for below: Maintains a professional environment in a multi provider, multidiscipline organization. Maintains a patient centered environment including but not limited to: Provision of care and advocate for patients of all ages and their families based on prudent nursing judgement and acceptable standards of practice. Efficient patient flow, assisting providers and other staff.REPORTS TO
: Chief Operating OfficerPRINCIPAL DUTIES AND RESPONSIBILITIES
: 1. Administrative and fiscal oversight of HIV and STI Clinic, Medical Case Management Services and Outreach activities. 2. Personnel management including hiring, performance evaluations, disciplining, and other activities 3. Assist in the identification of funding sources, preparation of grant applications. 4. Grants management ensure that all grant fiscal, administrative, data collection, and reporting requirements are met. 5. Maintain program expenditures within budget parameters. 6. In collaboration with COO, Director of Nursing and Director of Substance Use Services, develops annual plan for community involvement and outreach 7. Create and strengthen relationships with community partners, interfaith organizations, and government and state agencies. 8. Maintain program expenditures within budget parameters. 9. Work within a management structure to collaborate with other Supervisors and Program Managers. 10. Other duties as assigned.PROFESSIONAL BEHAVIOR

: Quality assurance monitoring•participates in the betterment of the health center through studies and reviews as necessary. Focuses on HIV Quality Metrics and works with the team to complete PDSAs to improve metrics. Helps educate staff on the importance of Infectious Disease screening within the general population and identifying high risk behaviors. Goal setting•assess personal, team, and organization goals. Participates in devising, reviewing, and revising policies, procedures and protocols.TEAMWORK ORIENTATION

: Bachelor of Science Degree in Nursing, Social Work, Public Health or related field preferred Knowledge of Infectious Diseases and standards of care Bilingual preferred.MINIMAL KNOWLEDGE BASE
: Advanced Computer skills including Excel, Word, Teams, and other Microsoft apps DPH Reporting requirements and software. Monthly reporting to funders on progress toward Work Plan goals.WORKING CONDITIONS
: Works in a busy community health center in a downtown business district. Long hours are sometimes required.PHYSICAL REQUIREMENTS
: Physical demands requiring lifting of small pieces of equipment, boxes, and /or patients not to exceed forty (40) pounds. Mostly standing and walking are required.
