What You'll Do As a PRN Home Health OT/COTA, you'll work directly with patients in their homes to provide individualized, high-quality occupational therapy.

Your responsibilities will include:

Deliver one-on-one occupational therapy treatments in home health settings Implement individualized plans of care under the supervision of the OT team Perform therapeutic activities, functional mobility training, ADL/IADL retraining, adaptive equipment recommendations, and home safety/fall prevention interventions Monitor patient progress and communicate updates clearly to the supervising therapist Educate patients and caregivers on home exercise programs, adaptive strategies, and safety Complete timely, accurate documentation in compliance with home health regulations Assist with discharge planning to ensure smooth care transitions What We're Looking For Active license in good standing Home health experience preferred (new grads with strong clinical skills are welcome) Ability to work independently and manage visits efficiently Strong communication and patient education skills Reliable transportation and willingness to travel Commitment to compassionate, high-quality patient care What We Offer ✨ Flexible scheduling — work when it fits your life ✨ Supportive leadership and responsive communication ✨ Clinical autonomy with therapist support ✨ Meaningful work with real patient impact We used Therapy Sync.
