Job Description
Job Overview We are seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) / Therapist to join our dynamic mental health team. In this vital role, you will provide comprehensive psychotherapy and clinical counseling services to diverse populations, at an in-home setting as well as office setting seeing adults and seniors. Your expertise will help clients navigate mental health challenges, develop coping strategies, and achieve their personal goals. This position offers an exciting opportunity to make a meaningful impact through patient-centered care, utilizing evidence-based practices in a supportive and collaborative environment. You will be responsible for conducting diagnostic evaluations, developing individualized treatment plans, and delivering various therapeutic modalities to promote mental wellness and resilience. Duties Conduct thorough patient assessments, including intake interviews and diagnostic evaluations using clinical research methods and medical terminology. Deliver individual psychodynamic therapy, behavioral therapy, applied behavior analysis (ABA), and crisis intervention techniques. Implement specialized therapies for clients with schizophrenia, active addictions with strong belief in harm-reduction model of care, PTSD, and many other serious mental illness diagnosis. Manage case documentation in electronic medical records (EMR) systems while ensuring compliance with HIPAA regulations and medical documentation standards. Collaborate with multidisciplinary teams to coordinate patient care plans, including social work support, medication management (psychopharmacology) with outside community providers, and any other specialized care providers in the individuals life. Utilize motivational interviewing skills to foster client engagement and facilitate positive behavioral change. Support individuals records through proper use and understanding of diagnostic coding processes such as

coding for accurate record-keeping and billing purposes. Participate in ongoing professional development to stay current with advances in mental health care, including trauma-informed care and outpatient treatment protocols. Experience Valid licensure as a Licensed Clinical Social Worker (LCSW) or equivalent in the state of practice. Could have a LMSW with supervision plan in place and willing to perform case management 8-10 hours per week. Proven experience providing psychotherapy across diverse settings such as outpatient clinics, hospitals, or community health programs. Demonstrated expertise working with addictions, schizophrenia, adults, geriatrics, or individuals with chronic complex co-morbidities. Familiarity with EMR systems for documentation and data collection purposes. Experience in crisis intervention and crisis management within behavioral health or hospital environments. Knowledge of DSM-5 relevant to mental health care and familiarity with CPT coding for billing procedures. Background working with special populations such as SMI and addictions is highly desirable. This professional will work alongside a team of community-based staff who provide skills, case management, and/or other services. Ability to design and perform effective psychoeducation curricula and lead effective psychoeducation groups and/or classes. Ability to work effectively as a team, interfacing with clients primary care providers, consulting psychiatrists and mental health specialists in the community, as well as with administrative and support staff. Ability to provide case management duties if applicable. Ability to exercise balanced judgment in evaluating situations and making decisions, and to handle difficult or confrontational situations in a calm, consistent and equitable manner.
