Job Description

•7am - 3pm / Monday - Friday Medication Cart LPN needed•Overview Join our dynamic healthcare team as a Licensed Practical Nurse (LPN) and make a meaningful difference in patient care! In this vital role, you will provide compassionate, high-quality nursing services across diverse settings, including long-term care, assisted living, hospitals, and community health environments. Your expertise will support patient recovery, comfort, and well-being through direct care, assessments, and collaboration with multidisciplinary teams. Responsibilities Deliver comprehensive patient care by performing physical examinations, vital signs monitoring, and health assessments. Administer medications and injections accurately following established protocols, including IV infusions and injection administration. Utilize electronic health record (EHR) systems such as Epic or PCC to document patient information meticulously. Assist with complex procedures such as catheterization, tube feeding, and sterile processing. Support patients with specialized needs such as dementia care, Alzheimer's care, memory care, and working with individuals with developmental disabilities. Collaborate with physicians and other healthcare professionals to develop and implement personalized care plans for acute and chronic conditions. Maintain infection control standards and ensure compliance with HIPAA regulations to protect patient confidentiality. Provide education on health coaching, medication management, nutrition, and discharge planning to patients and their families. Assist in medical imaging procedures or laboratory specimen collection when necessary to support diagnostic evaluation. Qualifications Valid state licensure as a Licensed Practical Nurse (LPN). Proven experience in various healthcare settings such as long-term care facilities, hospitals, hospice or palliative care units. Familiarity with EMR/EHR systems like Epic or PCC for accurate medical documentation. Strong knowledge of medical terminology, anatomy, physiology, ICD coding, CPT coding, and medication administration protocols. Experience working with diverse populations including geriatrics. Ability to perform physical examinations, venipuncture (phlebotomy), catheter care, and sterile techniques confidently. Excellent communication skills for effective patient assessment and interdisciplinary collaboration. Knowledge of infection control practices and basic life support (BLS) certification required; additional certifications like ACLS are preferred. $22.00 - $25.00 per hour
