Work Hours:

FT - 5x8s/wk, all shifts available, EOW, Holiday HCPs will float units at HLC - no floating to other facilities. EOW and Holiday required License Required /

Certificate:

NY LPN Licensure, BLS (cert must be through AHA or RQI specifically) Experience required: 1 year experience as LPN, Experience serving in an LPN capacity at LTC Facility Responsibilities Administer medications and treatments as prescribed by healthcare providers. Monitor patients' vital signs and report any changes to the supervising nurse or physician. Assist with medical imaging procedures and ensure patient comfort during examinations. Perform catheterization and other necessary medical procedures with precision and care. Provide support for botulinum toxin treatments as part of aesthetic or therapeutic procedures. Document patient information accurately using electronic health records systems such as Athenahealth. Collaborate with interdisciplinary teams in Level II Trauma Centers to provide comprehensive patient care. Educate patients and their families about health management, treatment plans, and wellness strategies. Maintain a clean and safe environment for patients, adhering to infection control protocols. Skills Strong knowledge of assisted living practices and home care nursing techniques. Proficiency in ICD-10 coding for accurate documentation of patient diagnoses. Excellent communication skills to effectively interact with patients, families, and healthcare team members. Ability to work in high-pressure environments, particularly in inpatient settings or trauma centers. Compassionate approach to patient care with a focus on empathy and understanding. Detail-oriented with strong organizational skills to manage multiple tasks efficiently. Familiarity with medical equipment and technology used in nursing practice.
