Job Type:
Full‑Time / Temp‑to‑Hire Job Summary P3SG is seeking a dedicated and experienced Licensed Practical Nurse (LPN)•Team Leader to support day‑to‑day nursing operations within a skilled nursing environment. This role requires hands‑on clinical care, strong documentation skills, and the ability to supervise CNAs while ensuring continuity and quality of resident care. The LPN Team Leader works closely with the RN Supervisor/Nurse Manager and the interdisciplinary team to support resident wellbeing, regulatory compliance, and a positive care environment. Key Responsibilities Provide direct nursing care to residents under the supervision of an RN Administer medications safely and accurately in accordance with policy Observe, assess, and report changes in resident condition with appropriate documentation Supervise and support CNAs to ensure high‑quality resident care Complete skilled nursing documentation, including change in condition, Medicare look‑back, admissions, transfers, and discharges Implement and follow resident care plans established by the interdisciplinary team Perform treatments, wound care, vital signs, and other clinical duties within the scope of practice Ensure infection control, PPE use, isolation precautions, and safety compliance Assist with pain management and resident comfort, including end‑of‑life care when applicable Communicate effectively with residents, families, physicians, and care teams Support admissions, transfers, hospital sends, and discharge processes Promote professionalism, teamwork, and a positive work environment Required Qualifications Graduate of an accredited LPN program Active Licensed Practical Nurse (LPN) license•State of New York Current CPR certification Skilled Nursing / Long‑Term Care experience preferred Strong clinical, documentation, and organisational skills Ability to supervise and lead CNAs effectively Basic computer skills and familiarity with electronic medical records Preferred Qualifications Prior experience in a Team Lead or Charge Nurse role Experience mentoring or orienting CNAs or new nursing staff Familiarity with electronic medical record (EMR) systems Additional Information Skilled Nursing / Long‑Term Care environment Full‑time opportunity with multiple shifts available No relocation assistance available No visa sponsorship available Only qualified candidates will be considered Apply Today First‑round interviews will be scheduled with our Chief Operating Officer, Lori McGuire.
