LPN (Licensed Practical Nurse) - SIGN ON BONUS (AM, PM, AND O/N SHIFTS AVAILABLE)

Responsibilities:

• Provide direct patient care, including administering medications, treatments, and performing procedures under the supervision of a registered nurse or physician.
• Monitor and assess patients' conditions, documenting changes and reporting to the healthcare team.
• Collaborate with other healthcare professionals to develop and implement individualized care plans.
• Educate patients and their families on health management, disease prevention, and treatment options.
• Assist with patient admissions, transfers, and discharges.
• Maintain accurate and up-to-date medical records.

Requirements:

• Valid LPN license in the state of Minnesota.
• Knowledge of human anatomy, physiology, and medical terminology.
• Ability to lift and move patients safely.
• Familiarity with long term care documentation practices.
• Proficiency with care of tube feeding, dressing changes, administration of IV antibiotics, and wound vacs.
• Proficiency in laboratory procedures and specimen collection.
• Strong interpersonal skills and the ability to provide compassionate care.
• Knowledge of health coaching principles for patient education and support.
• Experience in long term care settings is preferred.
• Familiarity with dementia care practices is a plus.
