Job Summary We are seeking a compassionate and detail-oriented Licensed Practical Nurse (LPN) for a Long Term Care facility in New Lisbon, WI . This is a 13-week contract position working 2:00 PM

• 10:00 PM, every other weekend .

The LPN will play a key role in medication administration, resident care, and communication with physicians and families. Responsibilities Administer medications safely while following the 5 Rights of Medication Administration Monitor residents and report any changes in condition to the charge nurse and physician Communicate updates to residents' families as needed Assist CNAs with call lights and daily care activities Report any concerns of abuse, neglect, or misappropriation to leadership immediately Maintain accurate documentation in the EMR (Meditech) Support the nursing team and perform additional duties as assigned Requirements Active Wisconsin LPN License (required) Current BLS/CPR Certification Long Term Care experience preferred Strong attention to detail and medication safety practices Excellent communication and teamwork skills Ability to work every other weekend Best regards
