Job Description
General Purpose The primary role of the LPN/LVN is to deliver direct nursing care to residents and supervise the day-to-day nursing activities of nursing assistants. Responsibilities must align with current federal, state, and local standards, regulations, and facility guidelines to ensure quality care.
Essential Duties & Responsibilities Note:

Administrative Functions Supervise daily activities of nursing assistants per regulatory standards. Enforce adherence to facility policies by nursing staff. Participate in departmental policy and manual reviews. Maintain updated nursing service procedures. Prepare reports and recommendations as needed. Coordinate care with other services and departments. Participate in surveys, QA programs, discharge planning, and documentation. Charting & Documentation Maintain records for admissions, discharges, incidents, and physician interactions. Record medication orders and update care plans. Ensure accuracy and compliance in documentation. Medication Administration Administer medications and treatments per physician orders. Maintain medication supply levels and narcotic logs. Ensure adherence to drug administration and disposal procedures. Personnel Oversight Assist with performance evaluations and staffing needs. Develop work assignments and provide shift leadership. Conduct regular staff meetings and enforce disciplinary actions fairly. Promote teamwork, training, and morale among staff. Nursing Care Functions Orient new residents and assist with admissions, transfers, and discharges. Collaborate with physicians on care plans. Deliver specialized treatments (e.g., catheterization, suction, dressing changes). Monitor and report changes in resident conditions. Support residents' physical and emotional well-being. Staff Development Plan and conduct in-service training sessions. Assist with staff orientation, standardization, and continuing education. Ensure staff is trained in exposure risks and safety practices. Safety & Sanitation Ensure staff compliance with safety protocols and infection control. Participate in fire and disaster drills. Maintain cleanliness of care areas and equipment. Equipment & Supply Management Recommend and maintain appropriate levels of equipment and supplies. Monitor safe equipment operation and prevent misuse or waste. Care Planning & Assessment Review and update care plans regularly. Ensure accurate nursing documentation and CNA compliance with care plans. Assist in MDS planning and documentation. Budget & Compliance Report suspected fraud or billing discrepancies. Support the QA & Assessment Committee with data as needed. Supervisory Responsibility The LPN/LVN has delegated administrative authority to manage their shift and supervise assigned personnel. Qualifications Education & Licensure Nursing degree or graduation from an accredited LPN/LVN program. Current, active, and unencumbered LPN/LVN license in this state. Current CPR certification. PCC (PointClickCare) experience preferred. Must maintain continuing education and remain in good standing with the State Board of Nursing. Skills & Abilities Proficient in English communication (spoken/written). Strong understanding of policies, procedures, and resident care documentation. Able to make decisions independently and work collaboratively. Must exhibit tact, patience, and professionalism. Strong organizational and leadership skills. Physical Demands Intermittent movement throughout the workday. Capable of lifting 25 lbs. to a height of 5 feet and moving it 50 feet. Ability to manage stress, sensory challenges (vision/hearing), and emergency situations. Must interact effectively with residents, families, and personnel of diverse backgrounds. Work Environment Work occurs in office and nursing care areas (e.g., nurse stations, resident rooms). Exposure to infections, odors, chemicals, and emotionally challenging situations.
