Position Summary/Responsibilities Case Manager Medication Maintenance staff will track and coordinate care for clients with mental health and co-occurring substance use and/or developmental disabilities. Staff will help ensure that access to medication as prescribed is maintained and help track those that need annual chart updates, yearly primary healthcare screening and referrals to treatment providers. Staff will ensure clients receive necessary and applicable services, accepting and closing cases as needed. Will also staff and coordinate transfers to higher levels of care as needed and document transitions in the electronic health record. Will assist with gaining access to outside healthcare records to encourage integrated healthcare efforts. Hiring Range The anticipated base salary for the Case Manager Medication Maintenance position is $43,022.07 to $45,890.21. Actual compensation offered within this range will be determined based on the candidate's experience, education, qualifications (knowledge, skills, and abilities), geographic location, complexity of role, and other relevant business considerations. We kindly ask that candidates do not discuss compensation until after an offer of employment is extended.
Requirements Education:
High School Diploma/GED required. Associates or Bachelor's degree in a human services field preferred.
Experience:
One (1) year of case management experience with population diagnosed with severe mental illness is preferable; previous work experience in a medical or behavioral healthcare setting very helpful.
Background Check:
Fingerprints, State and FBI criminal record reports, drug test, and central registry report will be required upon selection.
Safe Driving Record:
This position requires transporting clients or driving agency vehicles. Candidates must have a good driving record, which is defined as no more than two moving violations in the last three years and no criminal moving violations in the last five years. Special Work Conditions This role requires the ability to communicate effectively with staff, clients, and partners, both verbally and in writing. The employee must be able to remain in a stationary position for portions of the workday, move throughout office or community settings, and operate a computer and standard office equipment. The position may occasionally require lifting or transporting materials or supplies up to 50 pounds. This role may require participation in trauma-informed physical practices, such as standing, positioning, and de-escalation techniques, when such activities are essential to the position.
