POSITION TITLE
Quality Analyst
REPORTS TO
(TITLE): Quality/Risk Management Director
EMPLOYEE PAY STATUS
Non-Exempt
DESCRIPTION OF POSITION
Work as member of multi-disciplinary treatment team reviewing patient care and treatment provided in both inpatient and outpatient services by obtaining and processing data for the purpose of maintaining Quality/Process Improvement and Risk Management. Assists in assessing focus areas/units/departments for compliance with policies, procedures, and regulatory requirements. Proactively educates staff as well as monitor and report compliance data.
KEY RESPONSIBILITIES
:
Admissions/Discharges/Transfers:
Conduct chart reviews for compliance indicators.
Compliance/Regulatory Reporting:
Create/present compliance and regulatory reports as assigned.
QAPI:
Prepare and submit monthly data/reports to-from the hospital's QAPI committee.
Recordkeeping:
Maintains appropriate records of the Quality/Risk Department.
Training:
Provide staff in-service training and education. Perform other duties as assigned by leadership to support changing department needs, emergency coverage, and daily operational goals.
Minimum Requirements:
Education and Licensing:
High School diploma, required. Bachelor's degree in related area to Quality Assurance/Patient - Customer Service, preferred. Basic Life Support (BLS): Must obtain a current BLS certification by the completion of orientation, and maintain current status throughout employment. Management of Assaultive Behavior (MAB): Must obtain a current MAB certification by the completion of orientation, and maintain current status throughout employment.
Knowledge and Experience:
Three (3) verifiable years of direct quality / patient advocate experience, preferred Demonstrated knowledge of health care service delivery systems and quality improvement practices Experience working in acute care environment Ability to apply and interpret compliance and regulatory standards Strong understanding of throughput processes, facilitators and barriers in healthcare systems Familiarity with medical terminology, diagnostic terms and treatment modalities Knowledge of quality assurance data collection, reporting, and medical record-keeping requirements Ability to effectively facilitate various QA/Risk processes, includes but not limited to RCA, FMEA, etc. Knowledge of
CMS, TJC, CDPH
(Title 22), OSHA, and other applicable healthcare regulations
Skills and Abilities:
Maintains confidentiality of patients at all times. Ability to cope well with stress and have a strong sense of compassion. Proficiency with software and/or equipment (MS Office suite including Outlook, Word, Excel, PowerPoint). Strong organizational skills with ability to prioritize projects, work relatively independently, manage multiple tasks, and meet deadlines. Strong written and verbal communication skills. Collects data for Risk Management and Quality Assurance/Improvement in a timely manner. Audits, analyzes and ensures completeness of data. Makes preliminary analysis of Performance Improvement and Risk Management data. Collects and makes preliminary analysis of Perception of Care (Satisfaction) surveys. Collects and audits medical records for regulatory agency reports. Distributes reports to appropriate recipients in timely manner. Collects and audits occurrence records from approved sources (electronic and paper) for risk management data. Follow up and ensures proper, timely completion for incomplete risk occurrence report data. Orients new non-licensed staff concerning related standard work expectations related to risk management. Manages patient complaint intake and response log. Participates in assigned QAPI meetings and related events. Maintains completeness, accuracy and integrity of all assigned electronic and paper data for QAPI including maintenance (purging and saving). Ability to work independently and as part of a team. Good judgment, problem-solving and decision-making skills. Ability to work in a fast-paced, expanding organization.
Physical Requirements :
While performing the duties of this job, this position is frequently required to do the following: Use standard office equipment and access, input, and retrieve information from a computer. Use computer keyboard with manual and finger dexterity and wrist-finger speed sufficient to perform repetitive actions efficiently for extended periods of time. Communicate effectively in person or via telephone in a manner which can be understood by those with whom the person is speaking, including a diverse population. Give and follow verbal and written instructions with attention to detail and accuracy. Perform complex mental functions and basic arithmetic functions; interpret complex laws, regulations, and policies; collect, interpret, and/or analyze complex data and information.
Vision:
See details of objects at close range. Coordinate multiple tasks simultaneously. Reach forward, up, down, and to the side. Sit or stand for minimum periods of one hour at a time and come and go from the work area repeatedly throughout the day. Lift up to twenty-five (25) pounds.
