Job Description

Full-time, day shift, Monday-Friday, hours will vary No Weekends or Holidays •This position is on-site•
JOB REQUIREMENTS EDUCATION
Minimum:
High school diploma or GED/HSE accredited by the State Board of Education.
Preferred:
EXPERIENCE
Minimum:
One year experience in patient care. Proficient in Microsoft applications and electronic medical records.
Preferred:
LPN experience with two years in healthcare with prior abstracting experience with knowledge of quality initiatives. SUMMARY Assists with practice transformation efforts across primary care offices. Maintain knowledge and proactively identify new or changing quality metrics and initiatives so as to be a resource for information and interpretation for SMC. Responsible for accurate and timely reporting of quality data. Performs extensive medical reviews and chart preparation to ensure appropriate documentation, compliance with quality metrics, and care coordination for patients.
