Job Description

Build relationships. Welcome new residents. Support families. Tell our story in the community. Some healthcare jobs keep you behind a desk. This isn't one of them. We're looking for someone who can connect the dots between people, families, hospitals, referral partners and our community . One day, you're helping a family navigate the admission of someone they love. The next, you're talking with a hospital discharge planner, coordinating a resident's needs with our clinical team, or representing our facility in the community. And through it all, you're making sure residents and families know they have someone they can turn to. This is Social Services

+ Admissions

+ Marketing/Community Relations rolled into one important role.

WHAT YOU'LL DO SOCIAL SERVICES

— Be Their Advocate You'll help ensure the emotional and social needs of our residents are heard, understood and addressed. Build relationships with residents and their families Identify social and emotional needs and help develop appropriate plans of care Participate in resident assessments and discharge planning Help residents and families understand available resources and assistance programs Connect families with outside agencies and community resources when needed Work closely with Nursing, Administration and other departments Address concerns and help residents and families navigate the challenges that can come with long-term care

ADMISSIONS

— Make the First Experience a Good One Moving into a skilled nursing facility can be overwhelming. You'll help make the process easier. Communicate with prospective residents and families Coordinate and conduct facility tours Gather and complete required admission information and documentation Maintain awareness of available beds and pending admissions Coordinate admissions, transfers and discharges with Nursing and other departments Welcome new residents and help families understand what to expect Maintain relationships with hospital discharge planners, social workers and other referral sources

MARKETING & COMMUNITY RELATIONS

— Get Out There and Tell Our Story We don't want someone waiting for the phone to ring. We want someone who enjoys building relationships and creating reasons for people to remember us. Develop relationships with hospitals, physicians, healthcare professionals and community organizations Represent our facility at community events and meetings Help develop community-awareness and public-relations efforts Identify opportunities to strengthen referral relationships Help coordinate promotions, events and outreach Make sure information shared about our facility is accurate, professional and reflects who we really are

WHO WE'RE LOOKING FOR

This role needs someone who can comfortably move between a resident's room, a family meeting, a hospital referral conversation and a community event. A strong communicator and relationship builder Licensed Social Worker not required but preferred Organized enough to keep admissions and documentation moving Compassionate with residents and families Comfortable working with healthcare professionals and referral partners Able to handle sensitive situations with professionalism and tact Independent, resourceful and able to make decisions when needed Energetic about representing and growing our facility in the community Able to work collaboratively with Nursing, Administration and other departmentsHealthcare, long-term care, admissions and/or community-relations experience is strongly preferred. Education, licensure and experience requirements will be based on facility needs and applicable state and federal requirements.
