  • day introductory period with opportunity for continued/fulltime employment based on performance. Mildreds Helping Hands is seeking an energetic, professional Outreach & Referral Specialist to grow our referral network and connect inidviduals and their families with needed home
  • and community
  • based services.
This position will focus heavily on developing relationships with case managers, service coordinators, community organizations, veteran resources, and other referral partners. The specialist will represent Mildred's in the community, identify potential sources, conduct consistent follow up, assist with incoming referrals, and help convert qualified referrals to admissions.
Responsibilities:
Build and maintain relationships with community partners, agencies, and referral sources to drive client engagement Conduct outreach activities to identify and connect individuals with available services and resources Manage and track incoming referrals, ensuring timely follow-up and proper documentation Serve as a primary point of contact for community organizations, healthcare providers, and social service agencies Develop and implement outreach strategies to expand community awareness of available programs Attend community events, health fairs, and networking meetings to represent the organization Collaborate with internal team members to ensure smooth client intake and transition of care
Requirements:
Experience in community outreach, social services, case management, or a related field Strong interpersonal and communication skills with the ability to build trust across diverse populations Knowledge of local Indianapolis community resources, social service systems, or healthcare networks preferred Ability to manage multiple referrals and priorities in a fast-paced environment Proficiency in maintaining accurate records and documentation Compassionate, self-motivated, and committed to serving vulnerable populations Valid driver's license and reliable transportation for community outreach activities
