Full Job Description Qualifications & Requirements:
5+ years of professional experience, including at least 2+ years in outside sales, business development, or referral/account management (any industry) — candidates should have a demonstrated track record of prospecting, relationship-building, and meeting sales or referral targets. Prior experience in client services, care coordination, or healthcare administration is a plus, with home care or senior services strongly preferred. This is not an entry-level sales role. Strong understanding of non-medical home care operations and scheduling logistics. Exceptional communication, customer service, and conflict resolution skills. High proficiency in scheduling or CRM systems (experience with ClearCare/WellSky a plus). Comfortable with occasional after-hours or weekend responsibilities. Ability to work independently and as part of a team under time-sensitive conditions. Reliable transportation and willingness to travel locally for in-person visits or client meetings.
Reporting Structure:
Reports to Lucia DelVecchio for all marketing and community liaison responsibilities and training.

The majority of work will occur Monday through Friday, 9:00 AM - 5:00 PM. However, there will be occasional requirements to work outside these hours, including evenings andweekends, to accommodate client intakes, networking events, and meetings with referral partners.
Location:
This is a field-based position. The Community Liaison will spend the majority of their time in the field meeting with clients, families, and referral partners. Attendance at the Scottsdale office is expected for scheduled meetings, training sessions, and administrative work.

Professional appearance is required at all times, with the expectation that you are always prepared to meet a client or referral partner. Business Development and Client Relations Develop and maintain strong relationships with referral partners throughout the healthcare and senior services community, including physicians, hospital discharge planners, social workers, rehabilitation centers, skilled nursing facilities, assisted living communities, and other professionals who serve seniors such as attorneys, CPAs, and financial advisors. Represent Heritage Home Care in the community by conducting regular in-person visits, presentations, and networking events to promote services and generate qualified referrals. Collaborate with the broader marketing team to coordinate outreach efforts, share referral intelligence, and maintain consistent messaging across all Heritage marketing initiatives. Build and manage your own referral network within the company CRM, tracking all visits, leads, and follow-up activity. Respond promptly to all new inquiries and referrals originating from your referral network. Serve as the main point of contact for the clients and families you bring into the company. Conduct in-home or facility-based assessments, present service options, complete all intake paperwork, and create the initial care plan. Coordinate closely with the operations team to ensure a seamless handoff and timely start of services. Follow up with clients and families after services begin to ensure satisfaction and retention. Meet or exceed weekly and monthly activity goals related to in-person visits, referral follow-ups, and client conversions. Participate in weekly meetings with leadership to review referral activity, pipeline progress, and strategies for expanding Heritage's community presence. May be required to participate in the office on-call rotation, including covering phones and responding to client or caregiver needs after hours and on weekends.
Pay:
$45,000.00 - $65,000.00 per year
