Job Description
H ome Care 7Twenty6 is seeking a motivated, relationship-driven professional to expand our referral network throughout the Long Island and NYC Region. This is a filed-based role where you build relationships with healthcare professionals and community organizations to help families access quality home care. Responsibilities Build relationships with hospitals, physicians, rehabilitation centers, assisted living communities, social workers, case managers, discharge planners, senior centers, and community organizations. Develop and maintain referral partnerships. Identify new business opportunities through networking and community outreach. Represent Home Care 7Twenty6 at healthcare and community events. Maintain an organized referral pipeline using our CRM. Work closely with our clinical and operations teams to support an exceptional client experience. Qualifications Excellent communication and interpersonal skills. Self-motivated with strong organizational skills. Comfortable networking and working independently in the field. Valid driver's license and reliable transportation. Ability to travel throughout the Long Island and NYC region. Experience in healthcare, home care, marketing, business development, or community outreach is preferred but not required.
