POSITION SUMMARY
The Medical Coordinator is responsible for coordinating medical and psychiatric services for our adults with developmental disabilities along with supporting them in order to help achieve their personal outcomes.
DUTIES, RESPONSIBILITIES, AND ESSENTIAL FUNCTIONS
Ensures that the rights, health, safety and personal outcomes of the people living in the residence are supported at all times. Coordinates and schedules all medical and psychiatric services for those we support including, but not limited to routine care, follow up care, and emergency care. Confirms location of appointment, obtains vehicle for appointment, obtains directions to appointment and allows ample time for timely arrival at appointment. Ensures that all needed information for successful completion of the appointment is in hand prior to leaving for appointment (i.e., consultation form, medical book, nurses notes, etc.). Obtains completed consultation form and needed scripts from the physician prior to leaving appointment. Gives full attention to what medical professionals are saying, taking time to understand the points being made, asking questions as appropriate, and not interrupting at inappropriate times. Reviews medical appointment calendar on a daily basis to see what appointments are scheduled for the day. Completes proper documentation of treatment and ensures this is communicated to all appropriate team members. Maintains medication inventory and ensures medications are ordered as needed. Sets up and manages paper or electronic filing systems/databases, recording information, updating paperwork, or maintaining documents, correspondence, or other material. Operates electronic systems and coordinates the flow of information, internally or with other organizations. Ensures that all required documentation (paper and electronic) is completed /processed appropriately, in a timely manner, and meets quality and compliance thresholds. Performs other duties as assigned. Complies fully with FREE's confidentiality policies by holding in strictest confidence all information pertaining to members, residents, the people we support and their families, clients, providers, contractors, and employees. Meets all safety training requirements; adheres to all safety policies and reports any and all unsafe conditions; takes measures to prevent accidents and/or eliminate conditions that pose a safety or health risk. Coordinates, supports, attends and participates in all required events/ meetings/ reviews/ committees, including ones outside of standard working hours. Meets all job requirements and stays in compliance. Adheres to FREE's standard of ethical behavior as outlined in the Employee Handbook and Code of Conduct as well as applicable laws and regulations. QUALIFICATIONS
Education:
o High school diploma or equivalent required
Experience:
o Minimum of one (1) year of experience providing services to individuals with disabilities
Specialized Training, Licenses, Certifications:
o Valid AMAP certification required or must be obtained within first 120 days
Knowledge, Skills, Competencies, and Abilities:
o Proficient computer skills o Proficient writing skills with grasp of basic grammar and spelling o Demonstrated ability to work in a team environment with flexibility, keeping others informed and anticipating needs is essential o Must be efficient, well organized, and have excellent communication skills
Other Requirements:
o Must be at least 18 years of age or older o Valid driver's license and satisfy criteria to be an agency driver required o Based on business needs, loss of driving privileges may result in change of work location, shift, weekly hours, or termination of employment.
WORK ENVIRONMENT
This role is performed in-person. Travel between FREE locations and doctors' offices and other community locations required.
PHYSICAL REQUIREMENTS
Employee is required to sit, walk and stand for prolonged period of time (at least 8 hours). Employee is required to effectively communicate both verbally and in writing. Employee is required to operate standard office equipment such as computers, phones, photocopiers, fax and filing cabinets. Employee is required to reach with hands and arms. Employee is required to bend, stoop, kneel, crouch or stand on a stool as needed. Employee is required to be able to lift 50 pounds at a time.
