Job description Altru Rehabilitation Hospital, part of Altru Health System in Grand Forks, ND, is seeking a dedicated Physical Therapist to join our Healthcare & Medical Services team. In this patient-centered role, you will evaluate, plan, and provide evidence-based physical therapy interventions for patients across a range of diagnoses, including orthopedic, neurologic, post-surgical, and medically complex conditions. You will perform comprehensive assessments, develop individualized treatment plans, set functional goals in collaboration with patients and families, and deliver hands-on therapy to promote mobility, strength, balance, and overall functional independence. Working closely with physicians, nurses, occupational and speech therapists, and case managers, you will participate in interdisciplinary care planning, communicate patient progress, and coordinate safe discharges and transitions of care. You will educate patients and caregivers on home exercise programs, injury prevention, safe mobility techniques, and use of assistive devices, as well as contribute to ongoing quality improvement, clinical documentation, and outcomes tracking using our electronic medical record system.
