Job Summary:
Summary This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply. Responsibilities Located in the Rogue Valley, White City allows easy access to Metro-area of Medford while remaining engrossed with nature. White City is an unincorporated community in Jackson County, Oregon, in the Rogue Valley. It is about nine miles north of Medford and 34 miles north of the California border. Enjoy the scenic drives to either Crater Lake, Oregon's only National Park, and to the mountain resort Lake of the Woods. Take a family-friendly hike to a waterfall or ride the only section of the Pacific Crest Trail that's open to mountain bikers. Duties include but not limited to: Provides leadership, clinical expertise, and innovative approaches in managing complex primary care within the PACT model. Delivers sex-specific direct care to Veterans, ensuring individualized assessment, treatment, and follow-up. Coordinates care across the Primary Care Department, ensuring continuity, timely communication, and comprehensive support for Veteran needs. Manages assigned patient panel(s), monitoring overall panel health, engagement, and access to services. Utilizes clinical registries and data tools to track patient needs, identify care gaps, and ensure appropriate evaluation and follow-up. Promotes wellness through health education, coaching, and interventions that support disease prevention and chronic disease self-management. Collaborates in developing, implementing, evaluating, and revising policies, procedures, and clinical guidelines within the Primary Care service. Demonstrates leadership that positively influences care quality and operational effectiveness across the Primary Care Department and broader facility. Provides peers with constructive formal and informal feedback to support professional growth and enhance team performance. Shares primary care clinical knowledge to support colleagues, reinforcing safe, effective, and high-quality nursing practice. Contributes to a safe, supportive, and professional learning environment for all health care team members. Evaluates outpatient outcomes related to evidence-based practice changes at the individual, group, and population levels. Documents, monitors, and is accountable for PACT program or service-level outcomes that reflect broad, complex performance measures. Ensures that PACT outcomes align with organizational goals and demonstrate impact at any applicable level within the facility. Participates in continuous quality improvement efforts to advance patient care processes and outcomes throughout Primary Care. VA offers a comprehensive total rewards package:
