Job Description

Overview We are seeking a dynamic and compassionate Nurse Practitioner to join our multidisciplinary healthcare team. This vital role offers the opportunity to deliver high-quality, patient-centered care across our mobile primary care setting. As a Nurse Practitioner, you will leverage your extensive medical expertise to assess, diagnose, and manage patient health needs, ensuring optimal outcomes. Your proactive approach and dedication will contribute significantly to improving patient experiences and health outcomes in diverse populations. Duties Conduct comprehensive patient assessments including vital signs, physical examinations, and detailed medical histories to inform diagnosis and treatment plans. Manage acute and chronic conditions utilizing advanced clinical skills Perform diagnostic evaluations using laboratory tests, sonography, medical imaging, and specimen collection/processing to support accurate diagnoses. Develop and implement personalized care plans across various specialties Utilize electronic health record (EHR) for accurate documentation of patient encounters and medical records management. Provide telehealth consultations to expand access to care while maintaining high standards of patient privacy in compliance with HIPAA regulations. Engage in case management activities including discharge planning, utilization review, and working with patients with developmental disabilities or memory care needs. Educate patients on health coaching topics such as nutrition, medication adherence, disease prevention strategies including family planning or post-acute care instructions. Collaborate with multidisciplinary teams in hospital settings—including Level I & II trauma centers—and outpatient clinics to ensure seamless patient transitions from inpatient to home or assisted living environments. Maintain proficiency in CPT coding (Current Procedural Terminology), ICD coding (International Classification of Diseases), ICD-9/10 systems for accurate billing and documentation purposes. Support infection control practices through proper specimen handling and sterilization procedures while adhering to OSHA standards. Experience Candidates should possess substantial experience working across multiple healthcare environments. Experience with diverse patient populations (memory/dementia/dementia care), behavioral health cases—and working with people with disabilities enhances your suitability for this role. Additional qualifications include strong physiology knowledge; expertise in diagnostic evaluation; proficiency in CPT coding; experience with laboratory testing; understanding of HIPAA compliance; basic life support certification; nursing office experience; and familiarity with medical terminology relevant across multiple specialties. $120,000.00 - $160,000.00 per year
