Job Description

Company Overview Mosaic Health is a not-for-profit network of Community Health Centers dedicated to providing accessible primary medical, dental, and behavioral health care across multiple counties. Committed to serving underserved and underinsured populations, Mosaic Health strives to eliminate barriers to quality healthcare and promote community well-being through comprehensive services and outreach programs. Overview We are seeking a dynamic and experienced Nursing Lead to oversee nursing operations across our healthcare facilities. The ideal candidate will possess a strong background in clinical nursing, team management, and healthcare administration. As Nursing Lead, you will ensure the delivery of high-quality patient care, foster a collaborative team environment, and uphold compliance with healthcare standards and regulations. Responsibilities Administrative Directs personnel actions related to areas of responsibility including, but not limited to hiring, performance management, evaluations, staff development and disciplinary action of the nursing team members in conjunction with the COO and/or HR as needed. Implements continuing education and competency programs and services for the nursing team. Coordinates the resource needs for the clinical department, including maintaining an inventory of supplies, ordering supplies, maintaining and calibrating equipment and ordering equipment as needed and within budgetary constraints. Maintains interface with practice manager to coordinate nursing schedules and operational activities to ensure adequate staffing. Serves on community committees and task forces to address local issues and represents the needs and goals of the Center on external committees. Ensures compliance with federal, state and local agencies regulatory requirements as they relate to nursing staff scope of practice and competencies, licensure and continuing education requirements. Serves as the Employee Health nurse for the Center. The Nurse Manager will assure compliance with regulation and policy as they relate to the Mosaic Health's Employee Health program. Serves as a member of the Mosaic Health Quality team representing the Center's nursing and clinical care teams by reporting nursing quality program initiatives (incidents, complaints and patient satisfaction). The RN Nurse Manager supervises Registered Nurses (RNs); Licensed Practical Nurses (LPNs); and Medical Assistants (MAs).
Supervisory responsibilities include:
Instructing and training in work methods. Enforcing internal procedures and controls. Conducting performance evaluations. Assisting with problem resolution. Setting goals and expectations for performance. Organizing workflow and ensuring that employees understand duties and delegated tasks. Monitoring employee productivity and providing constructive feedback and coaching. Scheduling staff appropriately and reviewing/approving Paid Time Off (PTO) requests. Other duties as assigned
Nursing:
Works as a member of a multi-disciplinary treatment team and practices team-based care that is patient centered and holistic. Acts as a patient advocate. Performs nursing assessments and triage of patient care needs for new and ongoing patients indicating adequate knowledge of anatomy, physiology, and pathology. Obtains and documents medical history from the patient and/or significant other related to patient conditions or problems and develops a patient care plan as indicated. Provides education to patients, families, and significant others in planning care, establishing priorities, and setting/carrying out goals as needed. Collaborates with other health team members in the planning and implementation of patient care. Documents all encounters and pertinent information into the Electronic Health Record (EHR) according to organizational policies and procedures. Records vital signs and notes any change in EHR and pursues more-specific investigation as needed. Makes pertinent observation, using language that correctly describes body parts and functions, and draws logical conclusions that direct formation of a patient care plan. Participates in daily team huddles. Demonstrates an effective working relationship with others by accomplishing activities and/or objectives in a timely manner. Maintains and monitors the referral process assuring referrals are completed in a timely manner. ompletes Point of Care Testing (POCT) per provider order and documents results in patient's record for provider. Dispenses medication as directed by provider ensuring there is a written order for medications and/or injections per the 5 rights of medication administration. Properly stores and disposes of medication per regulation and Mosaic Health policy and procedure. Participates in the evaluation and peer review of nursing care given to patients as appropriate to Mosaic Health. Identifies strengths and weaknesses of nursing care systems and effectively communicates with COO. Identifies abnormal diagnostic data and shows an understanding of its implications by reporting information to the provider and others caring for the patient, and by incorporating information into the patient's care plan. Uses care plans as a basis for providing safe and therapeutic care to patients. Consistently follows established standards, policies, and procedures in providing nursing care. Completes nursing competencies with the assistance of the COO as needed. Monitors new employee orientation of nursing staff and assures Orientation Check List is complete. Informs COO of areas of concern related to the performance of individual nursing staff. Performs competently those technical skills considered basic to the type of care given at the Center. Identifies patient teaching needs and provides elementary instructions for the patient and his or her family. Seeks guidance from COO when encountering nursing procedures and responsibilities unfamiliar with. Incorporates discharge planning elements, and incorporates these in care plans, demonstrating an awareness of community care available after discharge. Responds appropriately in urgent and/or emergency situations. Enhances professional growth and development through participation in educational seminars, conferences, current literature, and in-service meetings. Uses telephone communication to assist and meet needs of patients, including provider follow up, prescriptions, and answering questions. Compliance/Service Accountabilities Collaborates with colleagues and team members. Follows and practices Mosaic Health's Mission, Vision and Values. Demonstrates ethical conduct and adheres to all Mosaic Health's policies and procedures. Maintains a positive attitude and professionalism under all circumstances. Complies with the health and safety requirements of federal, state and local laws. Maintains strict center and patient confidentiality in compliance with HIPAA. Attends and participates in required departmental/staff meetings/committees/conferences and/or retreats.
Minimum Qualification Standards:
Associate's or bachelor's degree in Nursing required. Current Licensure as a Registered Nurse in New York State. At least three years of management experience. BCLS certification required. Travel may be required between other office locations.
Required Knowledge and Skills:
Knowledge of medical terminology. Interpersonal and customer service skills. Excellent verbal and written communication skills. Knowledge of patient care charts and patient histories. Knowledge of appropriate procedures and standards for the administration of medications and patient care aids. Computer proficiency including e-mail, data entry, and generating reports using standard software applications.
Equipment, Machines and Software Used:
Ability to wear latex or vinyl gloves and operate a variety of office and medical equipment and instruments including, but not limited to wheelchair, syringes, needles, stethoscope, thermometer, scale, eyechart, oxygen machine, blood pressure cuff, sterilizers, nebulizer, POCT equipment, electrocardiogram, and Topcon camera.

Pay:
$80,000.00 - $89,000.00 per year
