  • Full-time Job Summary We are seeking a dedicated and compassionate Occupational Therapist to join our team.
The ideal candidate will have experience in various settings, including acute care, skilled nursing facilities, and outpatient clinics. The Occupational Therapist will be responsible for developing and implementing individualized care plans to enhance patients' functional abilities and overall quality of life. Responsibilities Conduct comprehensive assessments of patients' physical, emotional, and social needs. Develop personalized treatment plans focusing on improving activities of daily living (ADLs). Collaborate with interdisciplinary teams to ensure effective discharge planning and case management. Utilize knowledge of anatomy and physiology to inform treatment strategies. Work with individuals with autism and other developmental disabilities to promote independence. Document patient progress and modify care plans as necessary. Educate patients and their families on therapeutic techniques and home exercises. Maintain up-to-date knowledge of medical terminology and best practices in occupational health. Experience Proven experience in occupational therapy in a skilled nursing facility. Familiarity with discharge planning processes and case management principles. Strong understanding of patient care protocols and rehabilitation practices. Candidates should possess excellent communication skills, a strong commitment to patient-centered care, and the ability to work collaboratively within a multidisciplinary team.
