Job Description

Horizon PACE is excited to announce an amazing opportunity to be part of Horizon PACE's program (Program of All-Inclusive Care for the Elderly). This innovative program is for individuals 55 years or older, who are living with chronic illness, or disabilities and need coordinated medical care to continue living as independently as possible in their home and community. The healthcare team at Horizon PACE will approach coordinating medical, social, and home care services so individuals no longer need to manage their medical care alone. Horizon has over 25 years' experience providing care in Kentucky and is the largest provider of adult day health care services in the state. Horizon is opening 3 PACE locations in Kentucky, and we are excited to expand services to older Kentuckians. Under the supervision of the ADHC Director, the Health Care Associate is responsible for working with indirect supervision. Responsible for providing and assisting participants of Horizon with activities of daily living, restorative and supportive care. Assists with organization and implementation of activity program. Monitors health status and provides a safe environment for participants.
Position Responsibilities:
Responsible for Horizon participants during center hours, providing care in a manner which ensures participant satisfaction. Assists participants and or supervises with activities of daily living (bathing, dressing, grooming, hair care, mouth care, eating, hand and foot nail care and toileting) as assigned or needed. Creates and participates in group(s) and one-on-one activities in accordance with daily activity calendar and as requested by the Director of Activities. Monitors and assists participants with meals at feeding table. Assists, throughout the day, in maintaining clean and orderly day rooms, kitchen and personal care areas. Performs and or assists with laundry, including clothes for participants. Demonstrates dependability through consistent compliance with scheduled work hours. Notes changes in participant's physical or emotional health and communicates this information to the appropriate staff person. Maintains participants' medical record and fulfills Horizon PACE charting and reporting requirements. Follows assigned schedule, makes adjustments to meet unanticipated changes, while considering the needs of the participant and requirements of the Center. Operates equipment safely and in compliance with vehicle licensing regulations. Provides physical assistance to each participant when needed Follows all Horizon PACE program policies and procedures and applicable Occupational Safety and Health Administration (OSHA) safety guidelines. Maintains the confidentiality of all company procedures, results, and information about participants, clients or families. Participates in and supports Quality Improvement Initiatives. Participates in continuing education classes and any required staff and training meetings. Maintains any required certifications or licenses. Performs other duties as required or requested.
Knowledge & Abilities:

Is knowledgeable of participant rights and ensures an atmosphere which allows for the privacy, dignity and well-being of all participants in a safe, secure environment Supports, cooperates with and implements specific procedures and programs Supports and participates in common teamwork Assist supervisor with various tasks to support department operations as directed; may also be assigned special projects Perform tasks which are supportive in nature to essential functions of the job, but which may be altered or re-designed depending on individual circumstances Assists participants and caregivers in filing grievances
Experience & Education:
Must have a valid driver's license, proof of insurance and have means of transportation. High school diploma or equivalent preferred; Valid Kentucky Driver's License.
Preferred to have:
Have a minimum of one year's experience in working with the frail elderly.
