Patient Care Coordinator

Full Time PRN  Logan County Health Services Patient Care Coordinator The Patient Care Coordinator is the face of the Rehabilitation department! Candidates must be customer-service minded, greeting patients with positive energy, true empathy and genuine care while ALWAYS displaying the "patient comes first" attitude.

Essential Duties:

Answers telephone calls, takes messages or forwards calls; schedules, reschedules, or cancels patient appointments. Navigates rehab and hospital electronic medical record (EMR) for coordination of patient care, appointments, documentation, and billing. Files, copies, faxes, and processes paperwork and all electronic documents as appropriate. Organizes patient charts for therapists. Ensure completion of all required signatures and appropriate consent forms. Tracks clinical data for monthly reports and statistics. Performs pre-authorization for insurance plans and electronic insurance verification for each patient. Able to provide simplified explanation of insurance benefits to patients. A team player! Communicates efficiently and effectively with therapists, billing office, other LCHS departments, and outside facilities. Multi-tasks ALL aspects of daily clinic operations
