This vital role offers an opportunity to provide high-quality care in various settings, including assisted living facilities, group homes, and home health environments. Your efforts will promote independence, safety, and comfort for those you serve, creating a positive impact every day. Duties Assist residents or clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and toileting to ensure their comfort and dignity Monitor patients' health by taking vital signs, observing for changes, and reporting findings to healthcare professionals Support medication administration following care plans and HIPAA regulations to maintain privacy and safety Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary Maintain a clean and safe environment by performing light housekeeping tasks including cleaning patient areas and common spaces Use specialized equipment like Hoyer lifts or resident's to safely transfer individuals with mobility challenges Document care activities accurately in compliance with healthcare standards and assist in updating personalized care plans Experience Prior experience working in assisted living, nursing homes, or home & community care settings preferred Knowledge of patient observation techniques and monitoring vital signs effectively Hands-on experience with Alzheimer's care, dementia care, memory care, or working with individuals with developmental disabilities or autism spectrum disorders Familiarity with medication administration, care planning, and HIPAA compliance is highly desirable Certification in CPR and First Aid is required; additional certifications such as group home experience or hospice care are advantageous Ability to perform heavy lifting safely and work effectively with individuals requiring assistance with mobility or disabilities Strong communication skills for collaborating with healthcare teams, social workers, residents' families, and other caregivers Join us to provide compassionate support that empowers individuals to live their best lives.
