  • Part-time, Full-time, Contract Job Summary We are seeking a compassionate and dedicated Personal Care/Home Maker to provide essential support and assistance to individuals in their homes or community settings.
This role offers an opportunity to make a meaningful difference in the lives of those who need help with daily activities, health monitoring, and companionship. The ideal candidate will be energetic, detail-oriented, and committed to delivering high-quality care that promotes independence, safety, and well-being. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support Prepare nutritious meals tailored to individual dietary needs and preferences Monitor and record vital signs such as blood pressure, pulse, temperature, and respiration rates to ensure health stability Administer medications accurately following care plans and HIPAA confidentiality standards Support clients with mobility using equipment such as Hoyer lifts or transfer belts; perform heavy lifting when necessary Maintain a clean and safe environment through regular cleaning and organization of living spaces Observe and report changes in client condition or behavior, including signs of dementia or Alzheimer's disease Assist with light housekeeping tasks to promote a comfortable living environment Follow established care plans and document all services provided in accordance with healthcare regulations Provide companionship and emotional support to foster social engagement and reduce feelings of isolation Collaborate with healthcare professionals, family members, and support teams to ensure comprehensive care delivery Support clients during medical appointments or community outings as needed Skills Proven experience in home care, assisted living, or long-term care environments Knowledge of patient observation techniques and vital signs monitoring Familiarity with Alzheimer's care, dementia care, memory care, hospice care, and working with individuals with developmental disabilities or autism spectrum disorder Ability to perform CPR and First Aid confidently; certification preferred Experience with medication administration and understanding of HIPAA regulations for patient privacy Strong cleaning skills to maintain hygienic living spaces Ability to assist with ADLs efficiently while respecting client dignity and independence Excellent communication skills for effective collaboration with clients' families and healthcare teams Physical stamina for heavy lifting, assisting with mobility aids like Hoyer lifts, and supporting active clients Compassionate attitude combined with patience, empathy, and active listening skills Knowledge of home health practices including social work principles is a plus Join us in providing vital support that empowers individuals to live safely and comfortably at home.
