Job Description
The Personal Care Social Services & Admissions Coordinator plays a vital dual role in supporting residents and welcoming new individuals and families into our Personal Care households. This position combines relationship-centered admissions coordination with ongoing social services support, ensuring residents experience a smooth transition, meaningful engagement, and continued advocacy throughout their time in Personal Care. Serving as both a trusted resource for current residents and a knowledgeable guide for prospective residents and referral partners, this role is key to fostering a compassionate, responsive, and resident-focused community. Serves as a Social Services representative on assigned care teams in Personal Care. In doing so, builds relationships with residents, families, community professionals, and others as necessary to evaluate resident's personal needs and general environmental situation and communicates this in such a manner as to be utilized by the care team. Serves as a key member of the care team, working in partnership with other Social Workers and other care team members. Collaborate with the Personal Care and Communications team to support census goals through targeted outreach, relationship-building, and follow-up communication. Achieve and maintain budgeted census goals. Caseload includes all Personal Care residents. Assists with Personal Care admission processes Facilitates completion of admission paperwork timely. Ensures open resident rooms are always tour ready. Welcomes new residents and their families or support person, completes assessments timely and orients residents to care methodologies, procedures and delivery of services. Uses knowledge of the continuum of care and community resources to explain referral process and avenues for discharge planning. Ensures that social services documentation is properly prepared and documented timely in resident records in accordance with established procedures and regulations. Assists residents and families in level of care changes and the discharge planning process as needed. Collaborates with receiving Social Worker to assure adequate resident support. For discharge to home or another level of living, partners with other team members, physicians and residents in determining appropriateness of community resources and provides assistance in obtaining community agencies or resources for financial assistance, counseling and support to residents. Establishes and maintains contacts with community agencies and health care professionals in order to gather information, make referrals, and provide appropriate post-discharge care and support for residents. Visits and markets to outside community referral partners. Serves as liaison between Landis Homes' residents and community resources. Collaborates and consults with other care team members regarding resident progress through care plan meetings and informal discussions. Organizes Family and Resident Meetings with residents and families. Serves as a key leader of the Personal Care leadership team and rotates on-call. Complete DHS reportable reports when needed during on-call. Coordinate tours, inquiries, screenings, and admission documentation in alignment with regulatory requirements. Maintain and update the Customer Relationship Management system with all inquiries, managing interactions and potential residents and prospects (logging calls, emails and notes). Represent Personal Care services with professionalism and warmth in community outreach efforts, referral partner interactions, and internal campus collaboration. Contribute to periodic marketing initiatives and outreach activities based on census and organizational priorities. Maintain accurate and timely documentation consistent with Personal Care regulations and organizational standards. Promote resident autonomy, dignity, and quality of life through personalized support and engagement. Complete admissions assessments of prospective residents. Strong interdisciplinary collaboration Develops and maintains working relationships between the Social Services department and other departments, as well as consultants. Maintains awareness of current professional standards for Social Workers. Participates in committees as assigned in the continuum of care. Maintains awareness and participates in community planning related to the interests of the retirement community and services for residents and families. Keeps up-to-date in the field of social work/geriatrics by reading appropriate journals and books and by attending seminars, conferences and continuing education. Understands and models the Honoring Lives principles in leadership, relationships and customer service. Communicates calmly and gently with residents, family members and staff, exhibiting patience, compassion, understanding and sensitivity. Demonstrates dependability, competence, accuracy and is observant. Adheres to compliance policies regarding confidentiality, HIPAA regulations, and resident and client rights. Complies with regulatory agency standards and applicable policies and procedures. Complies with safety and infection control protocols in all aspects of the job. Demonstrates understanding of the Code of Conduct and abides by its guidelines in all aspects of the job. Attends in-services and team meetings as required for role. Additional duties and responsibilities as assigned. Bachelor's degree in social work or a bachelor's degree in a human services field including, but not limited to, sociology, gerontology, special education, rehabilitation counseling, or psychology is required. One year of supervised social work experience and/or one year of admission experience in senior living industry working directly with individuals is required. Admissions and marketing experience preferred. Adheres to National Association of Social Workers Code of Ethics, if applicable. Demonstrates ability to balance and manage challenges of a fast-paced environment. Demonstrates a pleasant personality with flexibility and adaptability for frequent interruptions and schedule variations. Able to work independently, well-organized, and can multi-task. Proficient in use of computer and related technology required for the role. Good verbal, non-verbal and written communication skills required.
