Physical Therapist

Job Summary We are seeking a dedicated and skilled Physical Therapist to join our healthcare team. The ideal candidate will be responsible for evaluating and treating patients with various physical conditions, developing personalized care plans, and ensuring optimal recovery outcomes. This role requires a strong understanding of anatomy, physiology, and medical terminology, along with experience in both inpatient and outpatient settings. The Physical Therapist will work collaboratively with other healthcare professionals to provide comprehensive patient care across different age groups, including pediatrics and geriatrics. Responsibilities Conduct thorough assessments of patients' physical abilities and limitations. Develop individualized care plans based on patient evaluations and goals. Implement physiotherapy treatments, including manual therapy techniques such as trigger point therapy. Monitor patient progress and adjust treatment plans as necessary. Educate patients and their families on exercises, mobility techniques, and home care strategies. Collaborate with multidisciplinary teams to ensure cohesive patient care. Maintain accurate documentation of patient records using EMR systems. Participate in discharge planning to facilitate smooth transitions from inpatient to outpatient or home health settings. Stay current with best practices in physical therapy through continuous education and professional development. Skills Strong knowledge of anatomy, physiology, and medical terminology. Proficiency in developing care plans tailored to individual patient needs. Experience in acute care, inpatient, outpatient, home health, and clinic settings. Familiarity with discharge planning processes. Ability to work effectively with diverse populations including pediatrics and geriatrics. Expertise in physiotherapy techniques including orthopedics, trigger point therapy and manual therapy. Excellent communication skills for effective patient education and collaboration with healthcare teams. Proficient use of EMR systems for documentation and patient management.
