Job Description
Job Overview- River Parishes Area We are seeking a dedicated and energetic Physical Therapist to join our dynamic healthcare team. In this vital role, you will assess, plan, and implement personalized rehabilitation programs to help patients regain mobility, reduce pain, and improve their overall quality of life. Your expertise will support patients in home health environments. This position offers an exciting opportunity to make a meaningful difference by applying your physiotherapy skills in diverse clinical contexts. Responsibilities Conduct comprehensive patient assessments to determine functional limitations and develop tailored care plans based on physiology and anatomy knowledge. Manage discharge planning processes by coordinating with multidisciplinary teams to ensure seamless patient transitions from hospital to home or community settings. Provide specialized care for geriatric, orthopedic, and neurological patients in home health environments. Document patient progress accurately using Electronic Medical Records (EMR) systems and communicate treatment plans clearly with patients and team members. Educate patients and caregivers on injury prevention, home exercise programs, and community resources to support ongoing recovery. Requirements Valid licensure or certification as a licensed Physical Therapist in the relevant jurisdiction. Experience in home care settings preferred. Strong knowledge of physiology, anatomy, orthopedics, geriatrics, and trigger point therapy techniques. Familiarity with EMR systems for documentation and patient management. Excellent patient care skills with the ability to develop effective care plans tailored to individual needs. Experience in discharge planning and working within multidisciplinary teams to ensure comprehensive patient support.
