Responsibilities Conduct comprehensive patient assessments to determine functional limitations and develop tailored care plans based on physiology and anatomy knowledge. Manage discharge planning processes by coordinating with multidisciplinary teams to ensure seamless patient transitions from hospital to home or community settings. Provide specialized care for geriatric, orthopedic, and neurological patients in home health environments. Document patient progress accurately using Electronic Medical Records (EMR) systems and communicate treatment plans clearly with patients and team members. Educate patients and caregivers on injury prevention, home exercise programs, and community resources to support ongoing recovery. Requirements Valid licensure or certification as a licensed Physical Therapist in the relevant jurisdiction. Experience in home care settings preferred. Strong knowledge of physiology, anatomy, orthopedics, geriatrics, and trigger point therapy techniques. Familiarity with EMR systems for documentation and patient management. Excellent patient care skills with the ability to develop effective care plans tailored to individual needs. Experience in discharge planning and working within multidisciplinary teams to ensure comprehensive patient support.
