Physical Therapist (Direct Hire) Location Grants, NM 87020 Schedule Full-Time Day Shift 8-hour shifts + on-call rotation Compensation & Benefits $110,000 - $130,000 annual salary Paid Time Off (PTO) Continuing Education opportunities Professional development support Comprehensive benefits package Potential relocation assistance Position Overview "Navitas Healthcare, LLC" is seeking a dedicated Physical Therapist to evaluate, plan, and administer individualized physical therapy programs for both inpatient and outpatient populations. This role focuses on restoring function, reducing pain, improving mobility, and preventing disability through evidence-based, patient-centered care. Key Responsibilities Patient Assessment & Care Planning Evaluate patients and develop individualized treatment plans Collaborate with physicians and interdisciplinary teams to establish care goals Implement evidence-based interventions to improve functional outcomes Therapeutic Interventions Provide manual therapy, therapeutic exercises, and mobility training Educate patients on ADLs, ambulation, and assistive device use Train patients on crutches, canes, prosthetics, and orthotics Treatment Administration Deliver modalities such as massage, traction, hydrotherapy, ultrasound, and heat therapy Supervise PT assistants, aides, and technicians as needed Progress Monitoring & Documentation Monitor patient progress and adjust care plans accordingly Evaluate prosthetic and orthotic fit and function Maintain accurate EMR documentation for compliance and continuity of care Discharge Planning & Continuity of Care Coordinate discharge plans with care teams and providers Develop home exercise programs and caregiver education plans Recommend outpatient therapy, home health services, and durable medical equipment Safety, Compliance & Professional Development Maintain a safe and compliant treatment environment Ensure proper maintenance of therapy equipment Participate in continuing education and professional development Support mentorship and interdisciplinary collaboration Required Qualifications Doctor of Physical Therapy (DPT) from an accredited program Active Physical Therapist license in the state of practice (or eligibility to obtain) Current BLS/CPR certification Minimum 1 year of experience preferred New graduates with strong clinical rotations encouraged to apply Strong knowledge of rehabilitation techniques and therapeutic modalities Excellent communication, clinical reasoning, and patient care skills For more details contact at hdavda@navitashealth.com or Call / Text at 516-862-1169.
