Job Description
Title Physician Assistant or Nurse Practitioner Summary Physician Assistants (PA) and Nurse Practitioners (NP) for Clinics, is responsible for providing patient care through assessment of health status, diagnosis, development and implementation of treatment plans, and evaluation of patient outcomes. This role also maintains accurate patient records, coordinates and administers follow-up care, and collaborates with other healthcare providers to ensure high-quality patient care. Compensation We are committed to providing compensation information for all open positions. The wage range for this position is $56.19 - $79.32 per hour. This role includes a base salary guarantee with additional productivity-based compensation opportunities above the base, which are determined annually. Productivity pay is reviewed annually and may be adjusted based on market conditions. Final compensation will be determined based on relevant experience, education, licensure, and internal equity considerations. Practice Details 64 hours per 2-week pay period (0.8 FTE) Standard hours:

11:00 a.m. - 7:00 p.m. Primary clinic location is the Northfield Clinic, with potential coverage at secondary off-site clinics as needed. Key Accountabilities Provide direct patient care for injuries and illnesses, including assessment, diagnosis, development, implementation, and evaluation of treatment plans based on comprehensive clinical evaluation of physical, psychological, emotional, societal, and environmental needs within PA/NP scope of practice. Order and interpret diagnostic tests, imaging, and therapeutic interventions as appropriate; determine follow-up needs and ongoing treatment plans; prescribe medications and monitor treatment effectiveness to ensure continuity of care. Provide patient education regarding diagnoses, treatment plans, preventive care, and healthy behavior changes to support improved health outcomes. Maintain accurate, timely, and complete patient documentation, including visit notes, assessments, treatment plans, and follow-up care, ensuring compliance with coding, billing, regulatory, and security requirements. Collaborate with physicians, nurses, and other healthcare professionals to coordinate care, facilitate referrals, and ensure safe, high-quality patient outcomes. Promote and support a positive, respectful clinical environment that upholds patient and staff dignity, privacy, and engagement. Ensure compliance with applicable laws, regulations, organizational policies, and healthcare service standards. Identify, report, and take appropriate action to correct unsafe conditions and support a culture of patient and workplace safety. Participate in clinic operations and support day-to-day workflow as needed, contributing to efficient and effective delivery of care. Engage in quality improvement initiatives, performance metrics, and organizational projects to enhance patient care outcomes and operational effectiveness. Collaborate effectively with multidisciplinary team members to foster a productive, supportive, and team-oriented work environment. Support organizational initiatives including operational improvements, compliance efforts, information systems initiatives, and other clinic-wide priorities. Other duties as assigned. Required Qualifications Master's degree in Nursing from an accredited Nurse Practitioner program (for NP candidates) Current Registered Nurse (RN) license in good standing with the Minnesota Board of Nursing (NP candidates) Current Nurse Practitioner license in the state of Minnesota (NP candidates) Current Physician Assistant License from the Minnesota State Board of Medical Practice in good standing (PA candidates) Certification as a Physician Assistant by the NCCPA (PA candidates) Must be enrolled inNH+C RQI
program for BLS certification and complete the requirements within 90 days of enrollment. Must remain current (complete quarterly assignments prior to the end of each calendar quarter). Complete required de-escalation and crisis prevention training upon hire and on an annual basis, including adherence to NH+C reporting standards, preparedness protocols, and response procedures.
