Position Overview:
We are seeking a compassionate and motivated Physician Assistant to provide primary care, rounding services, chronic disease management, assessments, and relationship-based care throughout senior communities. This role is ideal for candidates coming from primary care, hospital medicine, emergency medicine, internal medicine, geriatrics, urgent care, family practice, or post-acute settings looking for a more meaningful and patient-centered environment.
Key Responsibilities:
Conduct patient assessments and follow-up visits. Provide ongoing primary and preventative care services. Manage chronic and acute medical conditions. Collaborate with nursing staff, caregivers, and interdisciplinary teams. Support transitions of care and continuity efforts. Document care plans and maintain compliance standards. Build strong relationships with patients and families.
Qualifications:
Graduate of accredited Physician Assistant program. Active Minnesota PA license required. Strong communication and relationship-building skills. Experience in geriatrics, primary care, hospital medicine, or senior care preferred. Ability to work independently in a field-based setting.
