Job Description

Role Summary We are seeking a Nurse Practitioner to join a growing clinical team in a 0.8-1.0 FTE role (4-5 days/week) with flexible scheduling . This role includes delivering full-scope primary care, managing both acute and chronic conditions, and building strong relationships with residents, families, and interdisciplinary teams. Providers enjoy a high degree of autonomy, a collaborative environment, and stable, well-defined patient panels—all with no on-call responsibilities . Compensation & Incentives Base salary + RVU-based bonus structure (strong earning potential) Up to $15,000 relocation or sign-on bonus (based on candidate location) Benefits Flexible scheduling with autonomy over daily workflow Paid time off plus paid national holidays (prorated) No on-call responsibilities Annual CME stipend Employer-paid DEA and license renewals Medical, dental, and vision insurance options (based on FTE) 401(k) with employer match Malpractice coverage Opportunities for professional growth and leadership development Mission-driven, team-oriented culture Key Responsibilities Perform comprehensive patient evaluations, including physical exams and chart review Order and interpret labs and diagnostics; adjust care plans accordingly Diagnose and manage acute and chronic conditions Prescribe and manage medications per state and federal guidelines Educate residents, families, and staff on care plans and prevention Collaborate with clinical team members and facility staff to ensure coordinated care Maintain accurate documentation and support quality improvement initiatives Qualifications Current or pending Nurse Practitioner licensure (any state) Ability to obtain Minnesota licensure prior to start date (support provided if relocating) Board-certified or board-eligible Nurse Practitioner or Clinical Nurse Specialist Experience in geriatrics or a comparable care setting preferred Strong clinical foundation in primary and chronic care management Excellent communication and teamwork skills Commitment to patient-centered, ethical care Clinical Focus Geriatrics Primary Care About the Organization We are a mobile, facility-based primary care practice providing comprehensive, patient-focused care to residents in Skilled Nursing Facilities (SNFs), Long-Term Care communities (LTCs), and Assisted Living settings (ALFs).
