Primary Care Nurse Practitioner SNF/LTC (4-Day Week, No Call) + $15K Bonus

Role Summary We are seeking a Nurse Practitioner to join a growing clinical team in a 0.8-1.0 FTE role (4-5 days/week) with flexible scheduling . This role includes delivering full-scope primary care, managing both acute and chronic conditions, and building strong relationships with residents, families, and interdisciplinary teams. Providers enjoy a high degree of autonomy, a collaborative environment, and stable, well-defined patient panels—all with no on-call responsibilities .
