Job Description

Program Manager Job Description Clearwater Assisted Living is a rapidly growing assisted living organization with core competencies in assisted and transitional in-home care throughout Wisconsin. We serve adults with disabilities in the greater Western Wisconsin region. Our team strives to provide the best in-home care assistance available. The Program Manager is responsible for effectively overseeing their Community Supported Living program which includes leading a team of supervisors and direct support professional staff while managing the care of clients and their care teams. The Program Manager is a full-time, salaried position that will work Monday through Friday with the responsibility of managing the Rice Lake office location. The primary responsibilities of the Program Manager include: Client Management
  • Ensure that Clearwater Assisted Living is providing tailored services to meet the needs of our clients under your purview. The Program Manager is responsible for developing client goals and strategies to achieve these goals. Staff Management
  • The Program Manager will provide leadership and guidance to their team to ensure strategies are being executed during client shifts.
Coordinate new hire training, new client training and ensure that your staff are working confidently with their clients. Program Managers are responsible for creating the client/staff work schedule and changing that schedule as needed. Program Managers also adhere to any change in staff availability or time off.   • Program Manager oversees monthly turnover (the compilation of client reports, mileage logs, vehicle maintenance, etc), attend member centered plan meetings and curate monthly summaries for client teams to review. Program Growth
  • The Program Manager is responsible for growing their program with a manageable client load including building strong rapport and relationships with IDT's and seeking out referrals through those connections. The Program Manager will receive referrals, set up assessments and follow through with the process of onboarding clients. Office Management
  • the Program Manager is the point of contact for the office location, vehicle fleet and staff.
Community Supported Living is an ever changing, reflexive program. This list is comprehensive, but not exhaustive; there may be tasks added over time, as the program evolves and new ideas and systems are implemented to make Clearwater Assisted Living's Community Supported Living program the best Home Health Care service in the State of Wisconsin.
Major Duties and Responsibilities:
Client Management Correspond with Client Care teams and families Ensure Client outcomes are reached Conduct client assessments and onboard new clients as needed Participate in client meetings and reviews Review and update Individual Care Plans Staff Management Support Area Supervisors by providing guidance and insight to client challenges, staff questions and client goals. Ensuring your Area Supervisors are confident to perform their responsibilities independently Creating and maintaining staff and client schedules Train and lead your team to perform with our core values in mind Conducting performance reviews with staff Sit in on Area Supervisor interviews as available Administration/Program Growth Create and maintain an organized system within Google Drive Gaining and maintaining the Rice Lake area programs Seeking new clients (Emails and phone calls advertising the program and actively searching for new referrals) Ensuring client needs are met (Are we meeting the goals of the assessment and developing skills) Correspondence with Client IDT's (Emails, phone calls, general updates) Creating Individual Care Plans (ICP's) for incoming clients New Client Onboarding (Member schedule for services, total hours) Create, track, and update vital client documents (Appointment Form, Semi-Annual Assessment Reviews, Cancellation Tracker) Monthly Client Reports Managing and revising the staff schedule as needed Clearly communicate vehicle needs to our
Fleet Manager Qualifications:
Bachelor's degree in psychology, social work, healthcare, human services, human development and family studies, or a related field Ability to pass Caregiver Background Check Minimum of 2 years of relative job experience in a healthcare setting overseeing direct staff Valid State Driver's license with acceptable driving record
Compensation:
$65,000 starting annual salary
