Job Description

The Quality Improvement Specialist is responsible for monitoring, tracking, and improving clinical quality metrics across multiple medical office locations. This role supports providers and care teams in achieving quality goals, maintaining compliance with regulatory programs, and improving patient outcomes through data analysis, workflow optimization, and care gap management. Key Responsibilities Monitor and report on clinical quality measures including
HEDIS, MIPS, & CMS
Quality Programs Track provider and location performance trends and identify opportunities for improvement Maintain quality dashboards, scorecards, and reports for leadership review Extract, validate, and analyze data from Electronic Health Record (EHR) systems Identify care gaps and support patient outreach initiatives Collaborate with providers and clinical staff to improve documentation accuracy and compliance Educate providers and staff on quality measure requirements and best practices Recommend workflow improvements to enhance efficiency and patient care outcomes Support quality audits, regulatory reporting, and payer submissions Lead or assist with quality improvement and population health initiatives Monitor effectiveness of interventions and adjust strategies as needed Work with care teams to identify patients overdue for screenings, vaccines, and chronic care management services Assist with outreach campaigns through phone calls, letters, and patient portal messaging Qualifications High School Diploma or equivalent required Minimum of 2 years of experience in healthcare quality, population health, clinical operations, or related healthcare setting Experience working with multiple clinical sites preferred Experience with EHR systems and healthcare reporting tools preferred Skills & Competencies Strong analytical, organizational, and problem-solving skills Knowledge of
