Job Overview We are seeking a highly skilled and dedicated Registered Nurse to join our team. The ideal candidate will possess extensive clinical expertise, including experience in various healthcare settings such as hospitals, long-term care facilities, and outpatient clinics. As a Registered Nurse with UPLIFT HOMECARE, LLC, you will deliver exceptional patient care, coordinate treatment plans, and support our mission to enhance quality of life through personalized healthcare services. Responsibilities Participates in ongoing interdisciplinary assessment of the patient. Completes initial and ongoing comprehensive assessments of the patient's needs at appropriate time points as assigned. Participates in the development and evaluation of the plan of care in partnership with the patient, representative (if any), and caregiver(s). Contributes to the patient's plan of care/service plan. Administer medications, injections, and treatments following established protocols and safety guidelines. Monitor vital signs, perform physical examinations, and evaluate patient responses to interventions. Utilize EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems to document patient information accurately and efficiently. Provide education on health management and medication administration to patients and their families. Collaborate with interdisciplinary teams including physicians, therapists, and case managers to ensure coordinated care delivery. Maintain strict adherence to HIPAA regulations to protect patient confidentiality and privacy. Assist with specialized procedures such as IV infusion, suturing, catheterization, and sterile processing when required. Uses clinical decision making to efficiently and effectively manage individual patients. Participates in in-services or training, as requested. Qualifications Valid RN license with current registration in the state of Florida. Proven experience in acute care settings such as ICU or Level I Trauma Centers is highly desirable. Experience working with diverse populations including pediatrics, geriatrics, individuals with developmental disabilities, or those requiring hospice & palliative medicine. Proficiency in using EMR/EHR systems such as Cerner or Axxess for documentation purposes. Level 2
