Hospice Registered Nurse (RN)UpliftedCare | Mission-Focused Hospice Care The Role As a vital member of our Interdisciplinary Team (IDT), you will provide holistic care for terminally ill patients and their families. Your mission is to provide expert symptom management, comfort, and emotional support, ensuring every patient can experience their final months with dignity.

What You'll Do Assess & Plan:

Conduct systematic health assessments to identify and meet the physical and emotional needs of patients and families.

Deliver Care:

Implement and evaluate skilled nursing care based on the IDT's established Plan of Care.

Advocate:

Act as a dedicated advocate for patient goals and end-of-life wishes.

Collaborate:

Work independently while coordinating closely with a diverse team of healthcare professionals.

Requirements:

Current Illinois RN License. Hospice or community home health experience (preferred).

$75,000.00 - $88,000.00 per year
