Registered Nurse Everest Home Health LLC Windham, OH 44288 Part-time Part-time Overview We are seeking a compassionate and dedicated Registered Nurse (RN) to join our home health team on a flexible/PRN basis. The ideal candidate will be responsible for completing Start of Care assessments, opening new cases, developing individualized plans of care, and performing recertifications every 60 days. Responsibilities Complete Start of Care (SOC) visits and patient assessments Open new cases and develop individualized plans of care Perform recertification visits every 60 days Assess patient conditions and monitor for changes in health status Ensure all care is provided in compliance with state and federal regulations Accurately document all visits, assessments, and care plans in Axxess Communicate effectively with caregivers, patients, and agency staff Provide clinical guidance and oversight as needed Travel to patient homes as required Qualifications Active Registered Nurse (RN) license in the state of Ohio Experience in home health care preferred Strong assessment, clinical judgment, and documentation skills Familiarity with Axxess or willingness to learn Ability to work independently and manage a flexible schedule Reliable transportation and willingness to travel Excellent communication and organizational skills This position is ideal for a nurse who values flexibility and is passionate about delivering high-quality, patient-centered care in a home health setting.

Pay:

$40.00 - $55.00 per hour
