Registered Nurse

Overview We are seeking a compassionate and dedicated Registered Nurse to join our Hospice team. The ideal candidate will have a strong commitment to providing high-quality end-of-life care, focusing on comfort, dignity, and emotional support for patients and their families. As a Hospice Registered Nurse, you will assess patient needs, develop individualized care plans, manage symptoms, and collaborate closely with physicians, caregivers, and the interdisciplinary team to ensure compassionate and holistic patient care. Description Provides and directs the provision of home nursing care, based on agency policies and procedures, through the competent application of the nursing process.

JOB DUTIES/KNOWLEDGE

Provides skilled nursing care in accordance with the physician-approved treatment plan, including patient assessments, wound care, infusion therapy, and other specialized nursing procedures. Evaluates and updates patient care plans, monitors patient conditions, coordinates with healthcare professionals, and provides education to patients and families to promote independence and safety. Supervises home health aides and LPNs, maintains accurate and timely documentation, ensures compliance with agency policies and infection control standards, participates in QAPI activities, and assists with patient discharge planning.

QUALIFICATIONS

License in the State of Michigan Active BLS/CPR Certifications TB-test Driver's License Minimum of 1 year of nursing experience ( preferred ) Must have a Home Health care experience
