Job Description

Overview We are seeking a compassionate and dedicated Registered Nurse (RN) to join our Home Health team. The ideal candidate will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being. This role requires a strong clinical background, excellent communication skills, and the ability to work independently while managing a diverse patient population. Duties Conduct comprehensive patient assessments and develop individualized care plans. Administer medications and treatments as prescribed, including IV infusions and injections. Monitor patients' vital signs and overall health status, documenting changes accurately. Provide education to patients and families regarding health conditions, medications, and home care techniques. Collaborate with interdisciplinary teams to ensure coordinated care for patients. Assist with procedures such as catheterization, tube feeding, and wound care. Maintain accurate medical records in compliance with HIPAA regulations. Implement infection control measures and ensure a safe home environment for patients. Skills Candidates should possess a variety of skills to excel in this role, including but not limited to: Proficiency in EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks. Experience in critical care settings (ICU, emergency room) and knowledge of acute pain management techniques. Familiarity with medical terminology, ICD coding (ICD-9/ICD-10), and documentation practices. Strong patient assessment skills with the ability to perform physical examinations and triage effectively. Experience working with diverse populations including geriatrics, pediatrics, and individuals with developmental disabilities. Knowledge of hospice & palliative medicine, senior care practices, and behavioral health considerations. Competence in performing procedures such as phlebotomy, suturing, venipuncture, and airway management. Ability to provide health coaching and education on nutrition, medication administration, and disease management. Understanding of legal aspects related to workers' compensation law and NCQA standards. Qualifications:
Current and active Registered Nurse (RN) license in the state of [Georgia]. Graduate of an accredited nursing program (Associate or Bachelor's degree in Nursing). Minimum 5 year of nursing experience (home health, med-surg, or acute care preferred). Valid driver's license, auto insurance, and reliable transportation. Current CPR/BLS certification.
Preferred Qualifications:
Experience with EMR systems (e.g., Homecare Homebase, Kinnser, or similar). Familiarity with OASIS documentation and Medicare guidelines. Excellent critical thinking, time management, and communication skills. Comfortable working independently in varied home environments.
Working Conditions:
Requires travel to patient homes within a defined service area. Flexible schedule, but may include weekends, evenings, and on-call rotations. Must be able to lift and carry light medical equipment and assist patients as needed.
