Lawrenceville, GA •$80 per assessment and $50 for reassessment every 90/120 days. • Acti-Kare of East Gwinnett seeking a Registered Nurse (RN) to support our agency's clinical operations and ensure the highest standards of care for our clients. This is a flexible, per-client opportunity designed for nurses seeking additional income or a consulting-style role. In this position, you will serve as a vital link between non-medical oversight and compassionate home care, ensuring our services fully align with Georgia Private Home Care Provider (PHCP) standards. Primary Goals of the Assessment Evaluate physical, emotional, and social requirements. Create a tailored plan that addresses specific daily activities and concerns. Identify potential hazards in the home and recommend modifications. Provide the insights necessary to ensure compatibility between the caregiver and the senior. Key Responsibilities Conduct comprehensive in-home client assessments as needed to evaluate physical, emotional, and social requirements. Assess the level of assistance required for Activities of Daily Living (ADLs), such as bathing, dressing, and meal preparation Review and approve personalized client service plans, ensuring they address specific daily activities and safety concerns. Assist with maintaining strict compliance with Georgia PHCP regulations and standards. Audit client documentation for accuracy and completeness to ensure clinical and regulatory integrity. Ensure all services align with the senior's specific needs and identify potential home hazards. Review medication management, nutritional requirements, and hydration needs. Gain an understanding of the senior's typical daily schedule, personal preferences, and lifestyle Identify trip hazards, assess bathroom safety, and evaluate lighting and emergency preparedness Observe for signs of cognitive decline or emotional challenges to ensure appropriate support levels
Work Schedule & Flexibility Flexible Schedule:

As-Needed Basis:
Work is assigned on a per-client/per-assessment basis. Qualifications Active Registered Nurse (RN) license in the state of Georgia (Required). Strong clinical assessment and documentation skills are essential. Ability to work independently and manage time effectively. Previous experience in home care, long-term care, or a related geriatric field is preferred. Familiarity with Georgia PHCP regulations is a significant plus.
Pay:
$50.00 - $80.00 per day
