What You'll Do:
Identify hospice-appropriate patients through referrals, relationships, and community outreach. Educate patients, families, and providers on hospice services and care options. Conduct consultations and facilitate smooth transitions into hospice care. Build strong relationships with hospitals, senior communities, and referral partners. Support admissions and continuity of care efforts across the organization. Help drive growth while improving patient access to hospice services.
What We're Looking For:
Active Minnesota Registered Nurse license. Hospice experience preferred or strong background in ICU, oncology, palliative care, ER, care coordination, or discharge planning. Exceptional communication and relationship-building skills. Comfortable working independently and managing a field-based schedule. Reliable transportation and ability to travel throughout the Greater Twin Cities Metro Area.
