Job Description

The Nurse is responsible for providing direct nursing care to residents, assisting in the delivery of healthcare services, and supervising day-to-day nursing activities performed by nursing assistants, as appropriate to licensure and scope of practice. Responsibilities are carried out under the direction of the Director of Nursing and in accordance with current federal, state, and local standards, guidelines, and regulations to ensure the highest degree of quality care is maintained at all times. Essential Duties & Responsibilities Utilizes the nursing process to ensure assessments are performed in a timely manner and are appropriate for the resident's condition. Develops and updates care plans that address all resident conditions and concerns. Interventions are timely and appropriate, with reassessment and re-intervention occurring per policy and as indicated by the resident's condition. Recognizes emergencies promptly and responds appropriately. Administers medications accurately and within designated timeframes. Performs treatments and procedures skillfully and on time. Educates residents and families by assessing readiness to learn and barriers to learning. Delivers information at a level appropriate for the individual. Ensures education is complete and properly documented. Encourages and supports resident participation in decision-making regarding care and treatment. Applies knowledge of National Patient Safety Goals. Assures hourly rounding is completed, assessing toileting needs, pain, positioning, and ensuring possessions are within easy reach. Demonstrates awareness of fall prevention techniques by identifying residents at risk, communicating risks to caregivers, and reporting all falls or assisted falls promptly. Engages in infection control practices by utilizing personal protective equipment appropriately, performing diligent hand hygiene, and implementing proper isolation procedures when indicated. Completes documentation accurately and in a timely manner, reflecting resident condition, progress, treatments, and events. Practices within ethical, regulatory, and legal guidelines and all standards and legislation applicable to the position and licensure. Treats residents and families with courtesy and respect by introducing self, responding attentively to needs, assisting individuals awaiting attention, resolving concerns promptly, and answering call lights in a timely manner. Respects resident privacy and confidentiality by knocking before entering rooms, providing privacy measures as desired by the resident, and maintaining confidentiality of resident information. Maintains a professional appearance and demeanor by being well-groomed, adhering to dress code standards, communicating compassionately, and refraining from discussing facility business in the community. Creates a positive learning environment for students and new team members by demonstrating acceptance, tolerance, and a friendly, helpful attitude. Attends department meetings, in-service programs, continuing education classes, Relias modules, and other educational opportunities to remain current with practice changes, policies, and procedures. Demonstrates a willingness to learn and apply new knowledge while maintaining high standards of performance and openness to change. Assists in supervising and evaluating personnel under their direction, as appropriate to role and licensure. Reports resident concerns, incidents, and operational issues to the Director of Nursing or appropriate supervisor. In the event of a pandemic or emergency situation, job duties may be adjusted to support frontline staffing needs and ensure resident care needs are met. Performs all other duties as assigned.
