JOB DETAILS POSITION SUMMARY
•RN Care CoordinatorPOSITION DUTIES
•Oversee and streamline patient care by assessing needs, coordinating interdisciplinary services, facilitating communication among providers and families, and ensuring safe, efficient transitions throughout the hospital stay.MINIMUM REQUIRED QUALIFICATIONS
•CA RN license; BLS; ACLS; 2 years recent RN Care Coordinator experience in an acute hospital settingLENGTH OF ASSIGNMENT
•13 weeksSHIFT / HOURS PER WEEK
