• Case Manager / Utilization Review / CDI for an exciting Travel or Local job in Syracuse, NY.

Responsibilities:

Coordinate and manage patient care plans throughout hospitalization Perform utilization review to ensure appropriate level of care and resource use Collaborate with physicians, nurses, and interdisciplinary teams for discharge planning Review clinical documentation for accuracy, completeness, and compliance Identify CDI opportunities and communicate recommendations to providers Monitor patient progress and assist with transitions of care Document accurately in

EPIC EMR

Communicate with patients, families, insurance providers, and care teams Ensure compliance with hospital policies and payer requirements Participate in case reviews, audits, and care coordination meetings

Qualifications:

Active RN license (NY) BLS required 1.5+ years acute care experience Acute care case management experience required

EPIC EMR
