Full Job Description Job Summary We have openings available in the following Louisiana location: New Iberia, LA A qualified Registered Nurse (RN) candidate to perform the responsibilities of case management of home health care services to our patients based on a physician's plan of care. Responsibilities and Duties Coordinate multi disciplinary activities related to the patient's care plan Manage home health care personnel Process and manage authorizations for patient care services Document and chart data using electronic medical record system Complete timely documentation according to the rules of participation in medical service plans Qualifications and Skills One or more years experience in medical-surgical adult health nursing Experience in home health preferred but not required Must be experienced in managing care plans Experience with coordination of multi disciplinary activities and management of home health care personnel Self-starter with excellent organizational and writing skills Experience in current Medicaid, Medicare Regulations and Joint Commission standards home health standards helpful Has maintained a current and active registered nurse (RN) license in the State of Louisiana Experienced in electronic documentation Benefits Salary position with additional compensation: Mileage Reimbursement Per Visit Rates On-Call Pay New Admit compensation Benefits for Full-Time Employment include: Medical benefits - employer participation in costs Dental Vision Life Insurance Paid time off after 3 months of full time employment Holiday Pay Paid Vacation after one year
