As a Home Health Account Executive, you will match the capabilities of service locations to patient needs and facilitate patient flow from referral sources into Stonerise Home Health to achieve census and growth goals. Key Responsibilities Building and maintaining professional relationships with physicians, case managers, hospital and clinic staff, senior living communities, and other referral partners to educate them on home health services and identify appropriate referrals. Coordinating timely home health referrals by gathering clinical information, supporting eligibility assessments, and ensuring smooth communication between referral partners and the home health clinical team. Conducting community-facing education and outreach to increase awareness of home health philosophy, services, and benefits. Developing and executing territory growth strategies to increase referral volume and achieve monthly admission and census goals. Supporting families, patients, and caregivers by providing home health education, answering service-related questions, and helping guide them through the referral and admission process. Collaborating with the Interdisciplinary Team (IDT) to ensure accurate, up-to-date information regarding capacity, patient needs, and referral activity. Tracking referral trends, market opportunities, and provider engagement using approved CRM, EMR, or referral platforms. Participating in case conferences, team meetings, and strategic planning initiatives to support census growth and market development. Establishing, maintaining, and growing referral relationships to support market share expansion and achievement of census goals Ensuring all communications and interactions comply with HIPAA standards and organizational confidentiality policies. Representing Stonerise Home Health in a professional, compassionate manner that aligns with our mission, vision, and values.
